Health Forms

 

All new students and students entering grades 1,3,5,7,9, and 11 are required to have a physical done by a physician before the beginning of school. Students should have received the form, or you can find it here.

Also, please update immunizations as needed. Immunizations that are required include 4-5 diptheria, pertusis, tetanus (DPT), tetanus every 10 years, 4 polio, 2 measles, mumps and rubella (MMR), 2 hepatitis A, and 2 hepatitis B shots (all have an * on the physical form). Official records from a health care provider are needed to document all immunizations.

This helps assure that your students are healthy, allows them to travel on school trips, allows us to give them the best possible care and is a school board policy.
